Solicito participar en consulta(s) telef\u00f3nica(s) con personal m\u00e9dico de APS. Entiendo que las consultas telef\u00f3nicas se brindan como un servicio de conveniencia para m\u00ed. Entiendo adem\u00e1s que el personal del centro har\u00e1 esfuerzos razonables para brindar consulta(s) telef\u00f3nica(s) durante la respuesta a la pandemia de Wisconsin. Tambi\u00e9n entiendo que pueden dejar de brindarme consultas telef\u00f3nicas si no pueden satisfacer mis necesidades de esta manera.<\/p>

Entiendo que APS Medical solo ofrece consultas telef\u00f3nicas a clientes que residen en el estado de Wisconsin y que soy responsable de notificar al personal del centro si me mudo fuera de Wisconsin.\u00a0<\/p>

Entiendo que mi consulta ser\u00e1 con personal no m\u00e9dico del centro, incluyendo un defensor del cliente\/paciente y\/o un profesional licenciado. Tambi\u00e9n entiendo que las consultas con personal no licenciado no sustituyen la terapia y\/o la atenci\u00f3n m\u00e9dica ni el consejo de otros profesionales licenciados.\u00a0<\/p>

Entiendo que el servicio telef\u00f3nico utilizado por APS Medical no cumple con HIPAA. I request to participate in phone consultation(s) with APS Medical personnel. I understand that phone consultations are provided as a convenience to me. I further understand that the center personnel will make reasonable efforts to provide phone consultation(s) during Wisconsin\u2019s pandemic response. I also understand that they may discontinue providing phone consultations to me if they are unable to meet my needs this way. I understand that APS Medical only offers phone consultations to clients residing in the state of Wisconsin and that I am responsible to notify center personnel if I move out of Wisconsin.\u00a0 I understand that my consultation will be with nonmedical personnel of the center, including a client\/patient advocate, and\/or with a licensed professional. I also understand that consultations with unlicensed personnel are not a substitute for therapy and\/or healthcare or for the advice of other licensed professionals.\u00a0 I understand that the phone service used by the APS Medical is not HIPAA compliant.
